The signature of authorized person verifying hours worked by GoMed™ Healthcare personal warrant that time listed is correct and work was performed satisfactory. Signature acknowledges that client agrees not to employ or encourage employment of GoMed™ Healthcare personal for net 30 days following the last assignment. Client agrees to terms of net upon receipt. Time and a half are charged for 6 national holidays. 1.5% is charged on all accounts past due over 30 days. All court cost and legal fees will be the obligation of the client for the collection of delinquent account. 30 Minute lunch will be taken out of each shift automatically unless approved by the facility

*** All Shifts that are turned in by 10:00am central time will be paid the same day.
